Piatt County Nursing Home Foundation

The Piatt County Nursing Home Foundation supports the tradition of quality living for older adults served by the Nursing Home, its outreach programs and Maple Point. The Foundation is a 501(c)(3) charitable foundation established in 1986 for the purpose of receiving and administering gifts to the Nursing Home and its programs.

Donations to the Piatt County Nursing Home Foundation provide equipment, capital improvements, enhancements and “extras” for the direct benefit of residents. You can help the Foundation by contributing to the Annual Giving Program, donating through the Planned Giving Program or making a general donation.

The Foundation is dedicated to improving, maintaining, enhancing and broadening the services and programs of Piatt County Nursing Home for the benefit of older adults.

Your donation makes a difference in the lives of both the Piatt County Nursing Home and Maple Point Supportive Living residents, as well as the staff the serve our residents.

Tree of Caring

The Tree of Caring sculpture is a permanent tribute to those friends who have shown exemplary generosity to Piatt County Nursing Home. The Tree of Caring may also pay tribute to those whom you wish to honor, or as a remembrance to loved ones.

You can provide a visual expression of your support for Piatt County Nursing Home’s caring mission by having your name or memorial permanently engraved on a “leaf” or “stone” on the Tree of Caring.

Gifts of $500 (brass), $1,000 (silver) or $2,500 (copper) are recognized with an engraved “leaf.” Gifts of $5,000 (contributor) or $10,000 (benefactor) are recognized on a “stone” at the base of the tree.

Caring Angel Employee Recognition Program

Did someone at Piatt County Nursing Home or Maple Point go above and beyond and make a difference for you or your family member? You can pay tribute to that person who made your healthcare experience special. When you make a donation to the Caring Angel Program in his or her honor, your Angel will receive a Caring Angel lapel pin to wear with pride each day. We will keep the dollar amount of your gift confidential, and we will send an acknowledgement letter announcing that you have named the person as a Caring Angel who has made a difference.

Your gift will be used to support programs and services that will make a difference in the lives of both the residents and their caregivers.
